Thank you for taking the time to fill out this form before your pet's appointment. This ensures that we can optimize your pet's consultation with the veterinarian and have the latest information on their chart.
Our lobby is now open. Please arrive 10 minutes prior to your appointment time and check in with the front desk. If you would prefer to remain curbside for your visit, please text 503-232-3105 when you are here and let us know!
*IF YOUR PET IS COUGHING OR SNEEZING* please remain INSIDE of your vehicle when you arrive and call/text us at 503-232-3105 to check in. An assistant will be out shortly to escort you and your pet into an exam room. We want to take precautions to protect other pets in and around the hospital from possible airborne illness.
Face masks are optional for both clients and Rose City Veterinary Hospital staff however we are happy to mask during your appointment by request.
We have a cancellation/no show policy in place. If an appointment is missed or cancelled with less than one business day of notice, you will be charged for the price of a full exam.
