New Client Form

If you are visiting Rose City Veterinary Hospital for the first time, we will have you fill out a client information sheet upon your arrival at your first visit. If you would like to save time, we have provided the same form here for electronic submission. Just fill out the following information and choose submit. Remember, we are an appointment only facility, and you will also need to contact or Client Care Representatives via phone or email to schedule an appointment for your pet before coming in.
